PH crash after water change

I have a 100 gal planted tank. The ph is always at 7.1-7.2. I push co2 that goes on after the light do and is cut off before the lights ona timer (about a bps) I have had a few ph crashes over the years, the most recent being this weekend. I did a WC with R/O water and added the mineral buffer (2 teas per 5 gal) the same way I do every time I do a change.
I finished the change (about a 15 gallon change) at about 8pm and for the remaining 4 hours the lights were on, all seemed well-the fish ate before I went to bed. I woke up the next day to half my fish gone.
All my crashes (including this one) happened after the lights went out and after a water change.
Is it that I am replacing water with R/O water with a much different ph? Is there not enough minerals to buffer the water? Too much co2 in the water? Should I add an air stone at night?
Right now the remaining fish are breathing at the top of the tank.
I have been unsuccessfully keeping talks for a while now and need to know WTF I am doing wrong. So disapppointed in myself!

Check you still have CO2/bubbles

If not, read up on end of tank dump.

That small of a water change isn't going to cause a pH crash

And check your tank hardness too. Maybe add something to buffer actually in the aquarium water, not as part of the water change
 
How often do you do water changes?
 
I was told by someone I trust that my issue was that I should have added baking soda to the water as well, to buffer the ph

Was that based on readings of anything?

Baking soda would normally only be used if doing water changes with soft water and for ongoing tank water maintenance I would have something in the water system to buffer and add hardness where needed.

Changing 15% of the water should not be a cause of total breakdown of your aquarium unless there's a large, underlying issue already.
