BLUE worm or "medusa" worm help/identification

I have had a freshwater tank for almost 10 years. At one time i bought what a small aquarium shop called a "blue worm" or a medusa worm (which i know isnt correct). It was about 10-12" long and it was some sort of amphibian. It was blue as the name suggests and would eat shrimp pellets. It would have to surface every few hours to get air. It basically swam like a snake. Unfortunately, after a couple of years, the worm actually wiggled its way out of the aquarium and died... :(

I have searched the net a bit and havent been able to find anything about what the real name is for the blue worm... i have since moved away and the aquarium shop where i bought it has closed, so i havent been able to find the real name for the blue worm!

I now have a 6 year old and i tell her about the blue worm and she (and I) would really like to get another one! any info or leads would be great!

thanks for the response...

however it wasnt a dragon fish...

it had smooth skin (felt like a frog) no scales. It moved pretty much like a snake to swim. No fins...

i also believe it was pretty much blind (had very small eyes that looked non functional)
